If checked “② Application will be made for registration of the company information above and the bank account information below as the registered company number has not been obtained.” in "Certificate of Payment Via Employer""事業主払込に関する証明書", you can leave this field blank.
＊In case that you make online iDeCo application or iDeCo+ (contribution payment plan for small and medium-sized companies) is available at your company, you are required to fill in this section.

■ For Mutual Aid Association members
Please write the number shown in the "Certificate of Payment Via Employer (for Mutual Aid Association Members)""事業主払込に関する証明書（共済組合員用）" prepared by your employer.
If you are a Mutual Aid Association member, the registered company number is required.
