(Please have the deceased's basic pension number or participant code (login ID) ready when the bereaved family contact us.)
We will mail the necessary documents such as Procedure Guide for Retirement Benefits and an application form to the bereaved family.
If you have designated a beneficiary in advance, the lump-sum death benefit can be claimed by the designated beneficiary. If not designated, the beneficiary will be in the following order according to applicable laws.
(i) Spouses (including those who were in a similar relationship of common-law marriage at the time of death)
(ii) Children, parents, grandchildren, grandparents, and siblings who relied mainly on your income at the time of death.
(iii) Apart from those mentioned in (ii) above, a relative(s) who relied mainly on your income at the time of death.
(iv) Children, parents, grandchildren, grandparents and siblings who do not fall under (ii) above.
