(1) Sompo Japan DC Securities will send you a "Request for an Account Balance Statement of Defined Contribution Pension Plan".

(2)Please fill out the "Request for an Account Balance Statement of Defined Contribution Pension Plan" and return it to Sompo Japan DC Securities, with stamps worth 460 yen enclosed.
