How do I make an application to receive income deduction (tax refund) on my individual-type DC (iDeCo) contributions?

Answer

The full amount of contributions in an individual-type DC (iDeCo) is subject to income deduction.
Those who make contributions to iDeCo by themselves (debit from their bank account) are eligible to receive income deduction by making year-end adjustments or filing tax return.
 
When making year-end adjustments or filing tax return, a "Certificate of Payment for Small Enterprise Mutual Aid Premiums (iDeCo Deduction Certificate)" "小規模企業共済等掛金払込証明書(iDeCo控除証明書)" needs to be attached.
The National Pension Fund Association"国民年金基金連合会" send this document to customers around the end of October every year. A "Certificate of Payment for Small Enterprise Mutual Aid Premiums (iDeCo Deduction Certificate)" shows the amount of contributions paid for the year, including the amount scheduled to be withdrawn by the end of that year.
If the initial contribution to iDeCo will be withdrawn in and after October, or if the contribution amount will be changed, a "Certificate of Payment for Small Enterprise Mutual Aid Premiums (iDeCo Deduction Certificate)" will be sent from the end of November to the end of January.
